2023 entry form


Please read the following instructions carefully.

The online entry form is below the instructions. ALL entries must be submitted online.

Entries must be received by 11 PM on April 3rd. All photos must be received by April 4th. All fees must be received by April 10th. Entries missing these deadlines will not be accepted. 

You can prepare your answers before filling out the online form using the printable formThe Registrar will not accept paper forms. If you need help with the online form, ask questions via email to the address below or visit the March Sew Day at Maki House.

You fill out the entry form by answering the questions right on this page. Move between questions using the Tab key or your mouse. At the end there is a submit button. You can review and make changes to your answers up until you submit your entry. Make sure your email address is typed correctly. After you submit your entry, you will receive an email summarizing your answers and indicating any fees to be paid. Retain the email for your reference. Reply to the email to request changes, such as fixing typos, supplying new measurements, or withdrawing your entry. If you don't get the email, contact the Registrar at the address below.

Your entry is incomplete without a digital colour photo of the quilted item. The entry form and photo may be submitted separately as long as the photo is identified with your name and the entry title. Your item does not have to be complete at the time you submit your entry, but the photo must be recognizable as to pattern, colour and style. If you choose to send your photo by email, the easiest way is to reply to the summary email you receive and attach the photo. If you send it in a separate email to the Registrar, please specify your name and the quilt title in the subject line The photo must be received by April 4th; otherwise, your entry will be withdrawn.  

There are two types of fees:

Entry fees ($10) for submissions in the main categories (bed, wall, art, and miniature). If your entry fee is not received by April 10th, your entry will be withdrawn.

Written critique fees ($10) if you have chosen that option on the form.  If your written critique fee is not received by April 10th, your quilt will not be critiqued.

Please indicate your name and the quilt title on your payment. If you have submitted multiple entries, you may pay all fees in one payment but your name and every quilt title for which you are paying fees must be listed so the Registrar can associate the payment with the correct entries.

The preferred payment method is by e-transfer to entryform@ottawavalleyquiltersguild.org. Please put your name and the quilt title(s) in the e-transfer message as they appear on your entry form(s) .

The Registrar will also accept cash or cheques in person at the March Sew Day at Maki House. Please provide the amount of your payment, your name, and the quilt title(s) on a separate piece of paper.

Please ensure that your quilt is insured under your personal household policy. We will make every effort to protect your work, but we do not carry insurance to cover the value of the quilt.
